The free scan asks for your first name and email address and records your self-check answers, score, and whether you chose the adult or parent path. The contact and speaking forms collect the information you enter. Please do not include sensitive medical details in these forms.

How submissions work

Form submissions are sent through FormSubmit to the DoctorTechNeck contact inbox so we can receive and respond to them. FormSubmit is an outside provider. The free challenge checklist saves checkmarks only in your current browser's local storage; those checkmarks are not sent to us. This version of the site does not have an email subscription sequence or member account.

Other services

The site loads fonts from Google and may display video from YouTube. Those services may receive technical information when their content loads. Our website hosting provider also receives routine request information needed to deliver pages.

Your choices

You may use the free challenge without entering an email address. To clear locally saved checkmarks, clear this site's browser storage. To ask about a form submission or request deletion from our inbox, email [email protected]. We will respond to requests we can verify and fulfill.
